About The Position

The Outpatient Assessment Clinician is responsible for completing a full bio-psycho-social assessment, complete with a preliminary diagnosis and referral and linkage to treatment. Assessments are scheduled in advance and are offered in person or via telehealth for clients coming out of a hospital setting, residential unit, emergency department or calling of their own accord. Some follow-up is needed, such as notifying a referral source that a client did not show or assisting a client in linking with a recommended service/level of care. The Outpatient Assessment Clinician will function as a core member of the Outpatient Clinic and participate in weekly treatment meetings.

Requirements

  • Master's or Bachelor's LSW/LPC; LISW/LPCC.
  • Experience completing diagnostic assessments preferred.

Responsibilities

  • Collaborate & communicate with all staff to assure appropriateness and safety of programming for each client.
  • Assist with dangerous and potentially dangerous situations and take appropriate measures to assure client/worker safety.
  • Perform bio-psycho-social mental health and SUD assessments to provide a preliminary set of diagnoses.
  • Review and summarize any referral information received.
  • Document the client's reason for seeking treatment.
  • Conduct lethality screening.
  • List the client's current problems and symptoms.
  • Document any current or historic substance use history.
  • Record medication history and physical health history.
  • Document the client's mental health history, including any hospitalizations or outpatient visits.
  • Assess the client's family and social dynamics and refer clients to community programs upon discharge.
  • Assess for risk for all clients receiving assessment services and consultation as appropriate.
  • Make appropriate recommendations for level of care using the CSSR-S, ODADAS Clinical Protocols, DSM criteria and Netcare protocols.
  • Provide general case management services for clients using the follow-up process.
  • Make appropriate internal/external emergency and crisis intervention referrals.
  • Obtain sources of collateral information from family members, therapists, case managers, family doctors, etc.
  • Perform linkage of clients to other mental health agencies and to Netcare's outpatient clinic as appropriate.
  • Complete all required quality data scales, such as the OQ, and submit data within indicated timeframes.
  • Serve as a multi-disciplinary team member on the outpatient treatment team.
  • Assist in daily Clinic needs like answering the phone, the door, and faxing information.
  • Perform related duties as required.
  • Assure adequate, accurate and timely documentation.
