  • M.D. or D.O.
  • Must be board certified or Board Eligible in Orthopedic Surgery
  • Active Oregon Medical License or ability to obtain prior to employment
  • Active DEA or ability to obtain prior to employment
  • Sports Medicine Fellowship training able to do joints
  • Other sub-specialties will be considered
