About The Position

The OPWDD Incident Management Investigator at Hawthorne Foundation Inc. is responsible for conducting thorough investigations into incidents that may violate the rights and protections of individuals receiving services. This role involves ensuring compliance with state regulations, preparing detailed investigative reports, and collaborating with various stakeholders to uphold the agency's mission of supporting individuals with autism and other developmental disabilities.

Requirements

• BA/BS in related Human Services Field or licensure in a medically related field required.
• Experience working within OPWDD programs required.
• At least one year of experience in conducting investigations involving OPWDD programs.
• Completion of approved Justice Center investigative training course.
• Thorough knowledge of state regulations governing incident reporting (14 NYCRR Part 624 and 625, Article 20 of the Executive Law and Article 11 of the Social Services Law).
• Knowledge of Protection of Individual in Programs certified by OPWDD (Part 633 regulations) and the Justice Center PPSNA.

Responsibilities

• Conduct investigations into situations that may violate the rights and/or protection of people receiving services.
• Complete investigations, IRC Committee recommendations, and follow-up within mandated timeframes.
• Prepare investigative reports and forward completed reports to supervisor within required timeframes.
• Assist with internal reviews regarding incident management as needed.
• Ensure investigations are timely, thorough, and address investigatory issues.
• Identify areas of concern during investigations that need additional follow-up and/or reporting.
• Interface with the OPWDD Incident Reporting Management Application (IRMA) for reviewing investigative information.
