About The Position

The Case Manager (CM) coordinates care for unhoused and unsheltered patients in the AAMRC/Grace Healthcare Intensive Case Management program by building trust through personal interaction, collecting data related to clinical progress and outcomes measurement, completing patient assistance program paperwork, as well as placing and tracking referrals to professional services. The CM works closely with the program's professional and clinical staff and program partners to help improve patients' health outcomes.

Requirements

  • Understanding of community resources in Gainesville and the greater Alachua area.
  • Strong communication, trust-building, and problem-solving skills; the ability to work independently in the field and adapt to changing conditions.
  • Proficiency with data entry, electronic medical systems and tracking clinical metrics.
  • Ability to pass background check; compliance with occupational health and vaccination requirements.
  • Active Florida RN Licensure and 2-3 years of clinical experience.
  • Valid driver's license and reliable transportation; ability to carry supplies and equipment.
  • Experience working with homeless/unsheltered populations; familiarity with chronic disease management.

Nice To Haves

  • Experience in navigating community resources as well as filling out applications for patient assistance programs, financial assistance programs, and other referral-based services is preferred.

Responsibilities

  • Outreach and engagement: connect with and maintain contact with unhoused and unsheltered patients accessing services at Grace Marketplace and/or Grace Clinic; provide trauma-informed, culturally competent support.
  • Clinical assessment: conduct basic clinical assessment of patients including the taking of accurate vital signs, accurate weight measurement, POC testing as directed and verification of current medications; ensure timely reporting of aberrations or concerns.
  • Patient assistance program (PAP): complete and submit paperwork to obtain free medication if indicated, track application status and approvals, troubleshoot any processing delays, and maintain organized documentation.
  • Data and reporting: accurately record clinical progress and outcomes measurements; interface with HMS electronic health record system, SU F's EPIC platform, and data bases related to outside referrals to compile date; enter timely, complete data into HMS electronic health record system while maintaining confidentiality and HIPAA compliance.
  • Referrals and navigation: place and track referrals to medical specialists, medical imaging or specialized labs, local social service agencies (housing, transportation, food, ID/legal aid, benefits), follow up until resolution, and document outcomes.
  • Education and counseling: provide basic disease management education using motivational interviewing and strength-based approaches.
  • Collaboration: coordinate with medical providers and community partners through scheduled huddles, email communications and/or phone calls to ensure accurate and timely communication regarding patient care components among all partners.
