About The Position

In an environment of continuous quality improvement, the Ophthalmic Scribe is responsible for real-time clinical documentation and workflow support for ophthalmologists during patient encounters. This role focuses on accurate, timely, and compliant documentation of the medical record, allowing providers to improve efficiency, patient interaction, and clinical throughput. The Ophthalmic Scribe adheres to IJCAHPO-aligned ophthalmic documentation standards. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times.

Requirements

  • High school diploma or GED required.
  • Certified as an ophthalmic scribe through IJCAHPO, or willing to obtain within one year or hire required.
  • Basic Life Support (BLS) certification required.
  • Typing, computer knowledge (windows based), medical terminology, anatomy, physiology, grammar, punctuation, and good secretarial skills.
  • must be able to communicate written and orally with physicians.

Nice To Haves

  • Prior experience as a medical scribe or in an ophthalmic clinical setting preferred.

Responsibilities

  • Documents physician-patient encounters in real time, including history of present illness, review of systems, assessment, and plan, under direct provider direction.
  • Accurately records ophthalmic examination findings, diagnostic test interpretations, and treatment plans as dictated by the physician.
  • Enters orders, diagnoses, and procedures into the electronic medical record (EMR) as directed by the provider, in accordance with scope and policy.
  • Prepares and organizes patient charts prior to clinic visits to ensure completeness and efficiency.
  • Ensures documentation supports appropriate coding, billing, and compliance requirements without independently selecting codes.
  • Assists with room turnover and patient flow coordination as needed to maintain clinic efficiency.
  • Maintains strict compliance with HIPAA, patient confidentiality, and medical record integrity standards.
  • Communicates effectively with physicians, ophthalmic assistants, technicians, and administrative staff to support clinic operations.
  • Adheres to standardized ophthalmology terminology and documentation practices consistent with IJCAHPO guidelines.
  • Demonstrates professionalism, adaptability, and attention to detail in a fast-paced clinical environment.
  • Maintains professional appearance and conduct at all times.
  • Works collaboratively as a team member within the Ophthalmology Department and across the health system.
  • Effectively copes with typical job-related stress and competing priorities.
  • Maintains required competencies, training, and certifications as applicable.
  • Assumes all other duties and responsibilities as necessary.
