Requirements

• LIC-Pharmacist License - Tennessee
• Relevant Work Experience
• Bachelor's degree

Responsibilities

• Provide and coordinate patient care and clinical drug therapy management services to patients managed with outpatient parenteral antimicrobial therapy (OPAT) and complex (oral) outpatient antimicrobial therapy (CoPAT)
• Obtain and document pertinent information about the patient's medical condition including signs and symptoms of illness, past and present medications, microbiological data, laboratory data, adverse effects, and medication adherence.
• Ensure the quality, safety, and consistency of clinical Pharmacy services in assigned areas.
• Develops and implements clinical programs designed to enhance patient outcomes and promote the most cost-effective utilization of medications.
• Develops and implements educational programs that advance the professional development, knowledge and effectiveness of staff and clinical pharmacists.
