About The Position

The On-site Patient Advocate at ApprioHealth serves as the primary contact for patients, guiding them through the application processes for various federal and state programs, including Medicaid and Social Security. This role involves direct patient interaction, documentation management, and collaboration with healthcare professionals to ensure patients receive the necessary support and resources.

Requirements

• High School Diploma or GED required.
• 1 to 3 years of experience in a relevant field.
• Knowledge of social work principles and case management techniques used in the Medicaid program.
• Familiarity with federal regulations and state laws applicable to the Medicaid program.
• Advanced interviewing skills.
• Proficiency in Microsoft Access, Outlook, Excel, and Word.
• Understanding of record keeping principles and practices.
• Ability to interact with individuals from diverse socioeconomic, ethnic, and cultural backgrounds.
• Knowledge of programs and eligibility requirements for Social Security Administration, State Medicaid, and County Social Services.

Responsibilities

• Visit patients in the hospital to pre-screen for qualifying criteria of various programs.
• Maintain ongoing communication with various agencies throughout the application process.
• Handle documentation and computerized record keeping.
• Assess cases and utilize appropriate community resources for client services.
• Prepare clear and concise reports and correspondence.
• Use initiative and independent judgment within established guidelines to organize work and meet deadlines.
• Establish and maintain effective working relationships with contacts in the course of work.
• Communicate effectively in oral, written, and telephone interactions.
• Process high volumes of cases within specified timeframes.
