About The Position

UPMC Health Plan and Workpartners are hiring a full-time Onsite Health Coach to join the Life Solutions Onsite team. This role supports an external client, the City of Pittsburgh. The position follows a Monday–Friday schedule with primarily daylight hours, though occasional early starts (as early as 5 a.m.) may be required. You will support multiple client locations but will primarily work in one area of the city per day. Primary locations include Downtown Pittsburgh and the Strip District, with additional onsite work at other client sites throughout the Pittsburgh area. As a member of UPMC Health Plan's Commercial Health Management team the purpose of the onsite health coach is to improve the population health and productivity of an assigned employer group(s) by (1) maximizing the engagement of employee-members in health and wellness-related activities via efficient and effective face-to-face interactions; (2) providing onsite wellness education and programming, lifestyle health coaching and appropriate referrals; and (3) collaborating with other entities (e.g. account managers, wellness committee members, HR representatives) in maximizing the successful participation of employees in defined health and wellness programs, activities, and initiatives.The onsite health coach will create a strong social presence within workgroups and build trust and respect with employees through positive interactions, providing service, and maintaining employee privacy and confidentiality. The onsite health coach will use non-invasive biometric measurements (e.g. body composition, blood pressure and expiratory CO) and online tools to assist in engaging as many employee-members as possible in conversations around health risk status, readiness to change, and on resources available to support good health, well-being and quality of life. The ability to leverage important social influences within workgroups (e.g. administrators, department managers, supervisors, and peer social leaders) and to garner their support in engaging staff is critical. The onsite health coach identifies and works with healthy members who seek to maintain their health status, those with modifiable lifestyle risk factors (such as tobacco use, overweight/obesity, poor nutrition, and sedentary or stressful lifestyles), as well as members diagnosed with pre-conditions (e.g. pre-diabetes, pre-hypertension) and conditions (e.g. COPD, CAD) who seek to reduce their modifiable health risks and better manage their conditions by adopting healthier lifestyle behaviors.

Requirements

  • Master's degree in psychology, health education, nutrition, exercise physiology, counseling, social work, public health or a related field required OR are currently enrolled in a Master's degree program.
  • Two years of health promotion/coaching experience.
  • Should be able to demonstrate proficiency in delivering two lifestyle improvement programs via in-person groups as measured by standard clinical quality assurance review criteria.
  • Knowledge of or ability to learn a client-centered counseling approach that emphasizes nonjudgmental empathic interaction and collaborative development and implementation of behavior change plans- emphasis on motivational interviewing and cognitive behavioral techniques.

  • Detail-oriented individual with excellent organizational skills.
  • Strong oral communication and social networking skills.
  • Ability to conduct non-invasive biometric measures (body composition, blood pressure, and expiratory CO) according to protocol in a variety of settings, including public settings; and provide care and maintenance of the required equipment.
  • Intermediate proficiency with MS Word and Outlook and ability to learn additional computer-based documentation skills.
  • Must take The National Board Certified Health & Wellness Coach (NBC-HWC) Exam within 24 months of hire, and pass within 2 consecutive exam cycles.

Responsibilities

  • Reviews health risk assessment and biometric screening results with members and supports them in selecting risk-appropriate wellness activities, lifestyle improvement interventions, and/or in creating their own Personal Lifestyle Improvement Program.
  • Enters all required data in Health PlaNET in a timely fashion. Responsible for care of the computer and protection of personal health information as required by HIPAA.
  • Works closely with MyHealth at Work clinic staff; the site wellness account manager, wellness committee and HR Department in maximizing employee health and productivity outcomes.
  • Competently employs motivational strategies for encouraging individuals to explore motivation for change and enhance readiness.
  • Actively promotes employee participation in UPMC-based and site-specific health and wellness activities, and serves as lead and role-model by participating as appropriate.
  • Works with other members of the integrated health care teams to review member health information and, based on member's health status, risk factors and readiness to change, to recommend/utilize appropriate health interventions.
  • Works collaboratively within interdepartmental and interdisciplinary teams.
  • Provides individual and group lifestyle improvement interventions; engages with members in regular, in-person contacts over time to assist them in setting health and wellness goals, tracking progress, solving problems, and maintaining positive changes over time.
  • Engages members who have specified risk factors in the collaborative development of a personalized behavior change plan to address the risk factor(s) in conjunction with health management team when appropriate.
  • Provides timely feedback to members' physicians about members' progress toward health behavior goals when appropriate.
  • Performs non-invasive biometric measures, e.g. blood pressure, weight and body composition analysis, and expiratory CO measures and reviews results as requested by employees; and properly maintains equipment.
  • Defines health behaviors or concerns that would potentially improve health if targeted for change.
  • Develops and maintains competence in the use of health coaching skills and earns progressive certifications in each lifestyle domain.
  • Develops and maintain knowledge of site-specific, UPMC and community-based health and wellness resources.
  • Tracks member progress toward defined goals.
