Requirements

  • Master's degree in Social Work from an accredited institution.
  • Licensed to practice as a Social Worker in Washington, D.C.
  • 2-5 years of experience in a healthcare setting, preferably oncology.
  • Current CPR certification.
  • Effective communication with healthcare teams and patients.
  • Knowledge of community resources and age-specific care needs.
  • Commitment to professional development and best practices in social work.

Responsibilities

  • Deliver compassionate counseling to patients and families throughout their cancer journey, based on referrals.
  • Assess and identify psychosocial needs through collaboration with the Cancer Committee and physicians.
  • Facilitate support groups and participate in Cancer Center events to enhance the psychosocial well-being of patients.
  • Create and coordinate discharge plans for patients with complex needs, working closely with medical teams and families.
  • Provide consulting support for inpatient discharge planning, ensuring continuity of care.
  • Document all services in medical records and coordinate grant-funded transportation options.
  • Serve as a liaison for post-treatment services with external providers and payors.
  • Identify patients with unique needs and facilitate referrals for specialized services and financial assistance.
  • Conduct educational sessions for staff on oncology social work topics.
