About The Position

The Oncology Social Worker (MSW - Medical Social Services Advocate) at Logan Health's Cancer Care Program plays a vital role in supporting patients, families, and caregivers by addressing social barriers within the healthcare system. The ideal candidate will possess a Master's in Social Work and be committed to providing compassionate care to patients facing complex social situations related to their health.

Requirements

  • Master's Degree in Social Work.
  • Experience in a medical setting preferred.
  • Current CPR certification may be required.
  • Excellent interpersonal skills to manage sensitive situations with professionalism.
  • Strong computer skills, including Microsoft Office Suite and ability to learn new software.
  • Excellent organizational skills and attention to detail.
  • Critical thinking skills and ability to set priorities.
  • Effective verbal and written communication skills.

Nice To Haves

  • Social work certification in oncology (OSW) or willingness to obtain it.

Responsibilities

  • Address identified support and social needs of patients, including trauma, palliative care, and complex social situations.
  • Support patients and families in making difficult decisions related to hospital stay and discharge.
  • Work with the interdisciplinary team to address social determinants of health impacting discharge planning.
  • Partner with stakeholders to identify social barriers to discharge and provide solutions.
  • Complete thorough and accurate documentation in the patient's medical record.
  • Act as an active member of the healthcare team, making early referrals and communicating with post-acute partners.
  • Advocate for clients' rights to self-determination and access to supportive services.
  • Complete one organization-wide teaching event or department-wide training session annually.
  • Identify and assume responsibility for reviewing and updating a Case Management policy specific to Social Work.
  • Exhibit professionalism and ownership towards Care Coordination.
