About The Position

This team member will be responsible for clerical support and procedure scheduling, data entry, and providing support for procedure scheduling and the front desk staff. He or she will work cohesively with other navigators, physicians, and office staff. The navigator is part of a multidisciplinary team and is the primary conduit for communication between providers, patients, and families. The navigator will work with the clinical teams and providers to identify patients who may need help with transportation, medication assistance, appointment coordination, food insecurity, referrals, and rent or utilities assistance. Partnership with management on metrics, reporting, and patient experience is required.

Requirements

  • High school degree or equivalent required
  • Associates degree preferred

Responsibilities

  • Manages all communications with referring providers regarding access and availability of appointments
  • Aids patients with pre-visit planning, throughout during clinic visits
  • Assists with clerical and administrative duties such as calling patients to ensure appropriateness of referral to the correct surgical specialty
  • Verifies that any necessary documentation is completed prior to visit with health insurance carriers, benefits, and prior authorization
  • Facilitates and coordinates care from outpatient to inpatient across the care continuum and communicates directly with the patient and family, to ensure follow-up
  • Assesses patients for social barriers that may limit successful clinical outcomes
  • Aids to coordinate transportation services as necessary to ensure the patient can attend appointments as needed
  • Attends and participates in meetings
  • Addresses any continuity of care concerns
  • Ensures appropriate and timely documentation
  • Demonstrates a productive work ethic while on duty
  • Obtains needed outside records and shares with the appropriate members of the navigation team
  • Maintain patient tracking data as the patients progress through their health journey
  • Manages patient no show policy, and reaches out to patients to discuss noncompliance and works to ensure the patient gets back on track
  • Interacts with clinic staff and providers daily on patient progress, mitigating issues or non-compliance
  • Participation in interdisciplinary team meetings to discuss high risk patients
  • Works consistently as an active team player
  • Monthly data reporting and monitoring
  • Supports the hospital and clinic departmental goals
