Provides professional nursing care to oncology patients. Assists in the development and implementation of an individualized plan of care, including prioritizing and coordinating delivery of care. Prioritizes and completes follow up assessments, evaluations and modifies plan of care as needed. Coordinates supportive care that occurs between office visits. Assesses patient and family needs in regard to physical, emotional, and social aspects of their disease and/or treatments. Understands the side effects of chemotherapy. Assesses patient and assists with general symptom management. Documents patient concerns, patient messages, and care provided to the patient in the patient's medical record for physician's review. Monitors and recognizes changes in physiological and psychosocial status and takes appropriate action based on assessment of the patient and family and Oncology standards of care. This is an SEIU-UHW represented position.
