About The Position

The Oncology Nurse Navigator serves as a dedicated care coordinator throughout the cancer care continuum. This role assesses and addresses the physical, emotional, and social needs of patients, while working closely with healthcare providers and internal/external partners to remove barriers to care. ONNs provide education, improve access, and streamline communication to ensure a supportive, seamless experience for patients and their families.

Requirements

  • Graduate of an accredited nursing program (required)
  • At least 1 year of nursing experience (required)
  • Current RN license in the state of practice or compact license (required)
  • BLS certification through the American Heart Association (required)
  • OCN certification required within two (2) years of hire
  • Strong knowledge of oncology practices and patient navigation
  • Excellent verbal and written communication skills
  • Critical thinking and conflict resolution skills
  • Ability to prioritize, manage time effectively, and work both independently and collaboratively
  • Proficiency with electronic documentation and EMR systems
  • Compassionate, patient-centered approach with strong attention to detail

Nice To Haves

  • Bachelor’s degree in nursing (preferred)
  • Outpatient nursing and scheduling experience (preferred)

Responsibilities

  • Assess, plan, facilitate, and coordinate care to meet patients’ physical, emotional, and social needs
  • Advocate for timely, high-quality care by guiding patients through the healthcare system
  • Coordinate appointments, referrals, and services to ensure continuous care
  • Provide education on treatment plans, symptom management, and health maintenance
  • Maintain accurate documentation and tracking of patient progress and clinical outcomes
  • Reduce barriers to care through anticipatory guidance, insurance coordination, and community resource navigation
  • Collaborate with oncologists, specialists, and support teams to enhance patient experience and outcomes
  • Educate patients on individualized care plans to reduce unnecessary utilization and improve quality of life
  • Serve as a trusted resource for patients and families throughout the cancer journey
  • Perform RN scope duties, including telephone triage and clinical assessments
  • Participate in clinic activities and assist in care coordination and planning
  • Perform other duties as assigned
