About The Position

Central Maine Healthcare is an integrated healthcare delivery system serving 400,000 people living in central, western and Midcoastal Maine. CMH's hospital facilities include Central Maine Medical Center in Lewiston, Bridgton Hospital and Rumford Hospital. CMH also supports Central Maine Medical Group, a primary and specialty care practice organization. Other system services include the Central Maine Heart and Vascular Institute, a regional trauma program, LifeFlight of Maine's southern Maine base, the Central Maine Comprehensive Cancer Center and other high-quality clinical services. The Nurse Navigator will be responsible for coordinating service and identified patients through a complete health system by assisting with access issues, identifying resources, providing educational material, and developing relationships with service providers and community. The Nurse Navigator coordinates the care in a timely and efficient manner, identifying and eliminating barriers to care. The Nurse Navigator is a consistent care coordinator throughout the continuum of care and assess the physical, psychological, and social needs of the patient enhancing patient outcomes, increasing satisfaction, and reducing cost of care. The Nurse Navigator is responsible for community education and screening as well as monitoring community outreach and promoting survivorship.

Requirements

  • Current and valid state RN License.
  • Current BLS certificate upon hire and maintain current.
  • Minimum of two (2) years of nursing experience.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred.
