Requirements
- Applicants should have a current Michigan RN license and possess an associate degree from an accredited nursing school (bachelor's degree preferred).
- Basic Life Support (BLS) required.
- Annual Flu Vaccination or declination
Nice To Haves
- Minimum of five (5) years of healthcare experience (preferred)
Responsibilities
- Acts as a patient liaison and performs program participation selection, invitation, scheduling and outreach.
- Oversees all efforts related to guiding patients and families through the screening, enrollment and appointment process.
- Communicates with referring physicians regarding patient appropriateness.
- Performs patient follow-up management for patients already screened, uses databases to ensure appropriate follow-up appointments are scheduled and kept as recommended.
- Recommend and implement guidelines. Developments require tools, forms and training materials.
- Performa additional job duties as assigned.
