This job will assist the patient, family and care giver to overcome healthcare barriers (including but not limited to social, financial, or physical barriers.) Responsible for conducting on-going assessments of need and facilitate referrals to additional Support Services throughout the Oncology continuum of care. Using the nursing process and evidence based knowledge, the Oncology Nurse Navigator provides education and resources to facilitate informed decision making (by the patient and/or family). The Prostate Navigator will work with the patient to overcome barriers in their care. They will facilitate access into the health system and coordinate care as needed. The navigator will assess health literacy and work with the patient and providers to maintain compliance to the plan of care. Navigators assess SDOH and will make appropriate referrals to supportive care. Navigation promotes communication between the care team, patient, and care givers. The navigator will assist in problem solving at any point in the patient's journey. The ideal RN will have work experience with urology/prostate oncology experience.
