This is a role that requires a multi-disciplinary team approach to solving problems and patient challenges. SUMMARY: Coordinates and oversees high-risk, time-sensitive oncology care to ensure seamless continuity across the patient care continuum. This is an exempt, salaried position.

Requirements

  • Active Nevada nursing license, Licensed Practical Nurse or Registered Nurse license.
  • Previous work-related skills, knowledge, or experience.
  • Advanced prioritization and critical thinking skills, with the ability to manage multiple complex patients and competing demands based on clinical urgency and risk.
  • Excellent written and verbal communication skills, with the ability to effectively engage providers, leadership, patients, families, and external partners.
  • Strong collaboration and interpersonal skills, with the ability to work across multidisciplinary teams in a fast-paced, high-acuity environment using tact and professionalism.
  • Proficiency in electronic health records (EHR) and data systems, with the ability to track, document, and manage patient care activities accurately.
  • Ability to coordinate complex workflows and problem-solve proactively, ensuring timely resolution of barriers to care across internal and external systems.

Nice To Haves

  • Bachelor’s degree from an accredited nursing program.
  • Certification in nursing coordination or case management.
  • Bilingual (English/Spanish)

Responsibilities

  • Coordinates care for high-risk and complex oncology patients, including transplant and out-of-state referrals.
  • Tracks and ensures completion of time-sensitive diagnostics, treatments, and procedures.
  • Monitors patient care plans for adherence to clinical protocols and required milestones
  • Proactively identifies and resolves gaps in care, ensuring completion of critical diagnostics, treatments, and referrals in alignment with clinical protocols and care plans.
  • Serves as a clinical liaison between patients, families, providers, and external institutions, with a focus on complex cases requiring multidisciplinary and cross-system coordination.
  • Facilitates communication and coordination with transplant centers and other referral institutions.
  • Ensures necessary medical records, imaging, and documentation are obtained and shared appropriately.
  • Collaborates with providers to support timely ordering and execution of treatment plans.
  • Provides education and guidance to patients and families regarding care plans and next steps.
  • Supports coordination of multidisciplinary care across internal departments and external partners.
  • Prioritizes caseloads based on clinical acuity, complexity, and risk of delays in care.
  • Maintains accurate and timely documentation within the electronic health record.
  • Participates in process improvement efforts to enhance care coordination and workflow efficiency.
  • Serves as a clinical resource to team members regarding the coordination of complex oncology care.
  • Performs other duties as assigned.
  • Maintain a high level of professionalism and always project this to the Board, all team members (employed and volunteers), parents/patients, partners and stakeholders.
  • Create and maintain good working relationships with organizational leaders and team members.
  • Build relationships that will support patient needs and appeal to new patients and organizations.
  • Check emails and respond in a timely manner.
  • Participate in mandatory training.
  • Create and maintain a gossip free environment.
