The position involves managing resources through interdisciplinary collaboration to achieve optimal patient outcomes for oncology patients. The role includes coordinating research data, community physicians, and Beacon Health System resources into a seamless model of access and care that benefits patients, participating physicians, and family members. The individual serves as a hospital and Beacon Medical Group ambassador from the patient's first contact to follow-up post-treatment for coordination of care. Responsibilities also include providing education regarding services available to oncology patients and developing survivorship care plans for all eligible oncology patients.

  • Assist adult oncology patients as they navigate through the healthcare system.
  • Conduct initial discussions with newly diagnosed patients and families to determine needs or barriers that may influence their participation in care.
  • Network with physicians and other healthcare providers to ensure effective implementation of patients' plans of care.
  • Act as a liaison between the family and the inter-disciplinary team by educating and assisting with the understanding of the plan of care.
  • Provide support to patients with issues regarding insurance, transportation, child care, financial resources, language barriers, and education.
  • Initiate referrals within Beacon Medical Group and other community agencies to connect patients with necessary resources.
  • Collaborate with community agencies to coordinate referrals as needed.
  • Utilize interdisciplinary care conferences to plan interventions for patients.
  • Encourage patient compliance with medical instructions and referrals.
  • Report information regarding variance trends to the manager/director or appropriate Physician reviewer.
  • Coordinate the gathering and reporting of patient outcome information.
  • Develop and discuss a survivorship plan that outlines treatment received and follow-up guidelines.
  • Conduct health screenings and provide educational offerings in the community.
  • Maintain an active role in the Oncology Care Committee.
  • Compile and analyze data for generating reports for the Oncology Care Committee.
  • Manage patients under each individual oncology clinic and/or BMG clinic.
  • Provide direct nursing care for multidisciplinary clinics.
  • Coordinate the scheduling of necessary procedures and follow-up required.
  • Complete other job-related assignments and special projects as directed.
  • Utilize the list of analytic cases from the Cancer Registry to identify eligible patients for treatment summaries and survivorship care plans.
  • Deliver and discuss survivorship care plans with eligible patients and their families.
  • Successful completion of an accredited nursing program.
  • Current license to practice as an RN in Indiana.
  • Oncology Certified Nursing (OCN) certification required within two years of hire.
  • Minimum of five years of oncology nursing experience preferred.
  • Prior experience in a leadership role in a healthcare setting required.
  • Thorough knowledge of oncology clinical care practices, procedures, and techniques.
  • Knowledge of community agencies, services, programs, and resources available to assist patients and their families.
  • Knowledge of counseling techniques and ability to identify and utilize appropriate resources.
  • Demonstrated communication skills necessary to effectively counsel patients and families.
  • Interpersonal skills necessary to promote and maintain cooperative relationships with patients, family members, physicians, staff, and the public.
