On-Call Weekend LPN – Skilled Home Health
About The Position
Looking for weekend-only work that still uses your clinical skills and makes a real difference? SummitWest Care, a mission-driven non-profit home health agency, is seeking a Weekend On-Call Triage LPN to support patients outside of standard business hours. This role is ideal for an experienced LPN who wants supplemental income, autonomy, and meaningful clinical work - without a full-time schedule. You'll serve as the calm, knowledgeable clinical resource patients rely on when urgent needs arise after hours. This is a non-traditional, weekend-only position, providing on-call triage coverage and in-person visits when clinically necessary. The role is largely remote, with field visits as needed based on operational and patient needs.
Requirements
- Active Colorado LPN license (required)
- Strong clinical judgment and independent decision-making skills
- Confident communication and triage assessment abilities
- Reliable transportation, valid driver's license, and auto insurance
- Comfort working autonomously in patients' homes
Nice To Haves
- Home health experience preferred, HCHB experience a plus!
Responsibilities
- Provide on-call triage coverage from Saturday 8:00 AM to Monday 8:00 AM
- Respond to patient calls, assess clinical needs, and determine appropriate next steps
- Complete in-home visits when scheduled or clinically indicated
- Collaborate with RNs, caregivers, and care team members to address urgent patient needs
- Document triage calls and visits accurately and timely
- Work remotely when not completing field visits
Benefits
- $700 flat stipend for the full weekend on-call shift
- $28–$33/hour for in-person visit time (based on experience)
- Mileage reimbursement for all work-related travel
- Ability to work from home between visits
