On Call RN or LVN (7 on 7 off)
About The Position
We are seeking a compassionate and dedicated Hospice On-Call Nurse to join our team at Butte Home Health & Hospice. As an on-call nurse you are working under the guidance of the Triage (Tele) Registered Nurse who will determine when visits are needed. As an on-call nurse, you will play a vital role in providing high-quality care and support to patients and their families during non-business hours. This position requires flexibility, excellent communication skills, and the ability to provide timely response and assistance in urgent situations.
Requirements
- Valid nursing license in California and current CPR certification.
- Minimum 1 year of nursing experience, preferably in hospice, palliative care, or a similar setting.
- Strong clinical skills and knowledge of symptom management and pain control.
- Excellent communication and interpersonal skills, with the ability to provide compassionate care and support.
- Ability to make sound decisions and respond effectively in urgent and critical situations.
- Proficiency in electronic medical record (EMR) systems and basic computer skills.
- Compassionate and empathetic nature, with a genuine desire to make a difference in the lives of patients and families.
Responsibilities
- Serve as the primary visit nurse for home health & hospice patients and their families during nonbusiness hours, including evenings, weekends, and holidays.
- Conduct in-person visits and assessments as needed, addressing symptom management, pain control, and emotional support. Visits average around 2 per evening. A candidate wishing to get more evening hours may work with coordinators to schedule planned PRNs.
- Collaborate with the interdisciplinary team to coordinate patient care and ensure continuity of care during on-call hours.
- Administer medications and treatments as prescribed, ensuring compliance with all policies and procedures.
- Document all patient interactions, interventions, and outcomes accurately and in a timely manner.
- Provide emotional support and counseling to patients and families during difficult times.
