We are seeking an experienced and compassionate Per Diem RN to join us in this mission. Job Description RN Hospice Case Manager- Per Diem Performs comprehensive nursing evaluations. Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action. Initiates and manages plan of care, provides appropriate nursing follow –up to all patients, and instructs caregivers in managing patient needs. Coordinates all services provided to patients and families, as well as appropriateness of care, to ensure quality of care and services.
Requirements
- Education: Graduate of an accredited school of nursing.
- Credentials: Current RN License within CA.
- A current CPR Certification, Valid Driver’s License and auto insurance.
- Experience: Minimum of one year experience as a professional nurse within the last 3 years with strong clinical assessment skills.
- Skills: All nursing functions, written and oral communication skills, sympathetic attitude, customer services/relations, ability to work with dying patients, organizational and diplomacy.
Nice To Haves
- Bachelors in Nursing preferred.
- Certification in Hospice and Palliative Nursing desired.
