About The Position

BAYADA Home Health Care is looking for a compassionate and dedicated On-Call Registered Nurse (RN) to join our team in our Baltimore West Visits Office. This office services our adult clients on a per visit basis in homes throughout Howard and Carroll Counties. This is an On-Call Position. Current HH/Oasis are required. You will be asked to do visits after hours during the following times: 5:00 pm – 8:30 am Monday, Tuesday, Wednesday and Thursday into Friday morning at 8:30. 5:00 Friday to 8:30 Monday morning. One year prior clinical experience as a Registered Nurse is required. As a home care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home. Requirements

  • A current license as a Registered Nurse in Maryland.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.

Nice To Haves

  • Prior home care experience strongly preferred, but not required.

Responsibilities

  • Follow a designated care plan in accordance with patient's needs
  • Make home visits to clients in designated geographic territories
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures
  • Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
  • Follow up with, execute, and properly document doctors' orders
  • Perform client assessments as necessary
  • Case management and coordination
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
