About The Position

The On-Call Registered Nurse at Traditions Health is responsible for providing nursing care and support to patients with advanced illnesses during after-hours shifts. This role involves assessing patient needs, delivering care, and coordinating with interdisciplinary teams to ensure high-quality patient care. The nurse will work independently, responding to calls from patients and families, and will document all interventions and communications effectively.

Requirements

• Graduate of an accredited school of nursing.
• Valid license as a Registered Nurse.
• Current Basic Life Support certification.
• Minimum one year of recent professional nursing experience.
• Experience in hospice or home health is preferred.
• Reliable transportation and valid driver's license.

Responsibilities

• Respond to calls from patients, families, caregivers, and referral sources.
• Assess patient needs and provide appropriate nursing care and advice.
• Document all calls, interventions, and changes to the care plan accurately.
• Coordinate patient care with interdisciplinary teams and contracted vendors.
• Provide support to families during patient deaths and offer bereavement resources.
• Perform patient admission visits and intakes as needed.
• Participate in the agency's on-call rotation and cover continuous care hours when required.
• Utilize teaching, assessment, and intervention skills to maximize patient comfort and quality of life.
