Central City Concerns is a new, trauma-informed recovery facility in the heart of Portland and we're looking for compassionate, mission-driven Medical Providers to be part of it. As an On Call Medical Provider, you'll work closely with our Medical Director to deliver high-quality, person-centered care to individuals impacted by substance use, mental health challenges, and houselessness.   • Perform admission evaluations, including buprenorphine induction
  • Manage chronic and acute conditions
  • Partner with a multidisciplinary team
  • Be available for on-call medical support
  • Provide admission orders for all new patients, including orders for the administration of Buprenorphine
  • Determine and formulate health histories and perform physical exams on all newly admitted patients
  • Order, interpret and evaluate diagnostic tests to identify and assess patients' clinical problems
  • Formulate treatment plans based on available clinical data, and work cooperatively with patients to implement treatment plan
  • Conduct ongoing health care and clinical management of stable, chronically ill patients
  • Determine and manage minor and/or acute illnesses and initiate treatment
  • Conduct initial care for emergency situations such as illness or injury and initiate arrangements for continuing care
  • Collaborate with an interdisciplinary team regarding patient treatment
  • Prescribe medication or other forms of treatment as necessary
  • Keep appropriate records
  • Participate in compliance, quality assurance and quality improvement activities as directed
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2
  • Strong foundation in addiction medicine and mental health care
  • Experience with underserved and homeless populations
  • Active Oregon licensure (or eligibility) and DEA certification
  • Commitment to health equity, harm reduction, and collaboration
  • Graduation from an accredited medical school
  • Unrestricted licensure to practice medicine in Oregon
  • Current BLS certification
  • Minimum of 2 years' experience in primary care medicine preferred
  • Experience in urgent care and/or primary care practice
  • Demonstrated skills in addiction and/or primary care medicine
