Requirements

  • Graduate from an accredited Occupational Therapy program.
  • Current licensure in the state(s) of practice as required.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Responsibilities

  • Conduct a c omprehensive assessment & evaluation using subjective and objective measures.
  • Develop treatment plans and programs in accordance with physician orders .
  • Provide quality treatments , actively progressing goals to achieve highest quality of life .
  • Conduct screens and provide education to residents /clients , families, caregivers, and staff .
  • Coordinate seamless discharge s utilizing the full continuum of therapy and fitness services .
  • Provide accurate and timely documentation & billing in electronic medical record.
  • Demonstrate understanding and knowledge of payer sources and regulatory requirements.
  • Complete and document supervisory visits per regulations.
  • Communicate effectively with residents /clients , families , caregivers, physicians, referral sources, and interdisciplinary team .
