On Call Licensed Vocational Nurse (LVN) - Hospice

Our mission is to provide compassionate, high-quality care that enhances the lives of our patients and their families. We’re growing—and we’re looking for dedicated, experienced LVNs to join our team and help us build the best hospice agency in the Inland Empire. What You’ll Do As an On Call LVN, you’ll play a vital role in supporting patients during nighttime hours. You’ll work closely with RNs and physicians to ensure continuity of care and comfort for our patients.
Requirements
- Valid California LVN license
- Minimum 2 years of direct patient care experience
- Current CPR certification
- Reliable transportation with valid driver’s license and insurance
Nice To Haves
- At least 1 year of hospice experience preferred
Responsibilities
- Follow and implement the patient’s plan of care as directed by the RN
- Provide timely and accurate documentation
- Conduct assessments and report findings to the care team
- Collaborate with interdisciplinary team members
- Use medical equipment and supplies efficiently
- Engage in ongoing professional development
