Summary: Applying the principles of Systems of Care, the Supervisor of OhioRise Care Coordination is responsible for developing and overseeing a team of Care Coordinators who ensure and cultivate flexible, family-focused, community-based responsive services based on the High-Fidelity Wrap Around Model of care coordination for Tier II Moderate clients and Tier III Intensive clients in alignment with The Village Network’s Mission, Vision and Core Values.
