Foster and maintain ongoing relationships with family members, service providers, hospitals, courts, education system providers, and other identified resources to facilitate and implement the Wraparound process. Cultivate flexible, family-focused, community-based responsive services Develop and maintain the Wraparound Team, including coordinating and leading team meetings Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update plan as necessary Ensure family support and stabilization during crises Provide and document the initial and ongoing Life Domain Assessment Maintain all service documentation requirements, evaluation outcome requirements and data as required Provide services in a timely manner and in accordance with Plan of Care and/or Crisis Plan Utilize and monitor Flexible Funding and service coordination Obtain weekly reports from subcontracted providers Participate in after hours on-call response Attend Program staff meetings, supervision and any other meetings as required Participate in the Agency and Program CQI Peer review process Perform duties to reflect Agency policies and procedures and comply with regulatory standards Meet Agency training requirements Report all MUIs to Site Manager and Supervisor immediately Other duties as assigned

  • Bachelors degree in related field and two years of direct experience or five years of related experience
  • Must complete High Fidelity Wraparound model training within 12 months of employment.
  • Knowledge of community-area resources for youth and families
  • Effective written and oral communication skills, including meeting facilitation
  • Ability to manage time and multiple priorities effectively
  • Demonstrated organizational abilities
  • Ability to work effectively with individuals of a diverse background
