About The Position
We are looking for an Occupational Therapist/OTR at Sunnyvale Post Acute! This position offers Full Time, Part Time, and PRN opportunities. As an Occupational Therapist, you will be responsible for developing effective treatment plans and obtaining approval for services from referring physicians. You will treat patients according to the physician's treatment plan and assist the nursing department with training of Restorative Aides. Additionally, you will supervise Occupational Therapy Assistants in direct patient care and patient-related activities, ensuring compliance with the state practice act. Communication with your supervisor and other health team members regarding patient progress, problems, and plans is essential. You will also participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed. Your role will include recording evaluations, daily treatment notes, weekly progress notes, and recertification documentation in accordance with the Occupational Therapy Board State Practice Act and governmental and third-party payer requirements. You will instruct patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services. Participation in discharge planning is also a key responsibility, along with recommending and facilitating the ordering of necessary durable medical equipment for patients to promote independence in mobility, such as braces, walkers, canes, elevated toilet seats, and grab bars. It is important to report any problems with department equipment to ensure it is maintained in good working order.
