About The Position

Cheer Home Care is seeking an Occupational Therapy Case Manager in San Diego. This role involves collaborating with interdisciplinary team members to develop and implement individualized care plans for clients and their caregivers. The OT Case Manager will also provide constant feedback to clients, client representatives, and other healthcare providers. They are expected to work closely with the office team and possess excellent communication and problem-solving skills. The position offers an option to work one weekend day and have one weekday off.

Requirements

• Upbeat Personality
• Active California OT license
• Strong Computer Skills
• Ability to multitask, drive, take action
• Strong Organizational & Communication Skills
• Pass Live Scan Fingerprint Clearance (LIC 9163)
• Valid driver’s license and proof of insurance required
• Cell phone with Internet Access
• Proof of Negative TB results
• Willing to Work One Weekend a Month as Back-Up On Call
