About The Position

The PRN Occupational Therapist in a Skilled Nursing Facility (SNF) plays a critical role in enhancing patients' quality of life by helping them regain and improve their ability to perform daily activities. This position involves assessing patients' physical, emotional, and cognitive needs to develop personalized therapy plans that promote independence and functional recovery. The therapist collaborates closely with interdisciplinary teams, including physicians, nurses, and social workers, to ensure comprehensive care and optimal patient outcomes. Working on a PRN (pro re nata) basis, the therapist must be adaptable and responsive to fluctuating patient needs and facility demands. Ultimately, this role contributes significantly to patients' rehabilitation journeys, enabling them to return to their homes or community settings with improved capabilities and confidence.

Requirements

  • Valid Occupational Therapy License
  • 1 year of occupational therapy experience preferred
  • CPR certification
  • Valid Driver’s License & Auto Insurance
  • HomeCare HomeBase experience preferred

Responsibilities

  • Perform clinical assessment; identifies patient needs and appropriateness of patient.
  • Documents, prepares, revises, and completes a Plan of Care, progress notes, phone orders and other clinical record documentation on an ongoing basis and in a timely manner.
  • Accepts clinical assignments that are consistent with education and competence to care for patients/clients.
  • Participates in the IDG care planning process.
  • Apply concepts of infection control and standard precautions in coordination/performing patient care activities to prevent contamination and transmission of disease.
  • Assess and facilitate through direct therapeutic intervention the patient's ROM, strength, coordination, need for and use of adaptive equipment and environmental modifications, functional mobility and safety issues as related to patient’s ADL performance needs. Fabricate and maintain adaptive equipment and hand splints.
  • Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
  • Evaluate patient occupational performance in the areas of self-care, work, leisure and rest. Assess sensory-motor neuromuscular, psycho-social and cognitive-perceptual components of occupational behavior. Assess physical, social and cultural components of the patient’s occupational environment.
  • Meets mandatory continuing education requirements of the Agency and the licensing board.
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.
