Requirements

Active Occupational Therapist state license
Valid drivers’ license
Two or more years of applicable occupational therapist experience

Responsibilities

Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
Conduct evaluations and develop occupational therapy treatment plan for our patients
Deliver treatment/care in accordance with care plan
Prepare and maintain accurate and complete clinical and progress notes on a timely basis
