About The Position

An Occupational Therapist in our PACE Program helps participants maintain independence and safely manage daily activities. They assess each participant’s physical, cognitive, and functional abilities, then develop personalized plans to improve skills such as dressing, bathing, cooking, and mobility. OTs recommend adaptive equipment, home modifications, and safety strategies to prevent falls and injuries, while also addressing cognitive and fine-motor challenges. They work closely with the interdisciplinary care team to support participants in remaining safely at home and engaged in meaningful daily activities.

Requirements

  • Master’s or Doctorate degree in Occupational Therapy from an accredited program
  • Current state licensure as an Occupational Therapist
  • Certification by the National Board for Certification in Occupational Therapy (NBCOT)
  • Knowledge of geriatric care and rehabilitation principles
  • Ability to assess functional, cognitive, and physical abilities
  • Strong documentation and communication skills
  • Ability to work collaboratively within an interdisciplinary healthcare team

Nice To Haves

  • Experience working with older adults or frail populations
  • Previous experience in a PACE, home health, rehabilitation, or long-term care setting
  • Knowledge of adaptive equipment, home modifications, and fall-prevention strategies
  • Experience with cognitive impairment or dementia care
