About The Position

Occupational Therapist (OT) – Full-Time Garden City Health Care Center is seeking a compassionate and skilled Occupational Therapist (OT) to join our dedicated rehabilitation team. General Purpose The Occupational Therapist evaluates and treats residents, collaborates with families, physicians, and interdisciplinary team members, and maintains accurate clinical documentation. The OT also provides education and in-services to facility staff to support optimal resident outcomes.

Requirements

  • Bachelor’s degree in Occupational Therapy required
  • Current, active California Occupational Therapist license in good standing
  • Must meet all continuing education requirements per state regulations
  • Proficient in electronic documentation systems (Casamba and PointClickCare preferred)
  • Strong clinical judgment, communication, and organizational skills
  • Ability to work independently and collaboratively
  • Frequent standing, walking, reaching, pushing, and pulling
  • Ability to exert up to 50 pounds frequently and 100 pounds occasionally
  • Occasional climbing, balancing, stooping, kneeling, crouching, or crawling
  • Visual abilities include close, distance, color, peripheral, and depth perception

Nice To Haves

  • Master’s or Doctorate degree in Occupational Therapy preferred
  • Skilled nursing or geriatric experience preferred

Responsibilities

  • Evaluate residents within 24 hours (or one business day) of physician referral; document any delays appropriately
  • Develop individualized treatment plans and obtain physician approval
  • Treat residents according to physician-approved plans of care
  • Supervise Certified Occupational Therapy Assistants (COTAs), aides, and students in compliance with state practice act
  • Assist nursing staff with training of Restorative Aides as appropriate
  • Communicate resident progress, challenges, and plans with the rehab and interdisciplinary teams
  • Participate in patient care conferences, utilization review meetings, and rehab conferences as needed
  • Complete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirements
  • Educate residents, families, and caregivers on maintenance programs and discharge preparation
  • Participate in discharge planning and care coordination
  • Recommend and facilitate ordering of durable medical equipment (DME) to promote independence in activities of daily living
  • Report equipment issues to ensure safe and effective operation
  • Maintain positive, professional relationships with residents, families, staff, and colleagues
