About The Position

Apollo Home Healthcare is hiring a full-time or part-time/per diem Occupational Therapist to join our Home Healthcare Team and see patients in Northern California's Bay Area. In this role, you will provide high-quality occupational therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will supervise a Certified Occupational Therapy Assistant (COTA) and work with a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care. Requirements

  • Graduate of an accredited Occupational Therapy program (Master’s or Doctorate in Occupational Therapy)
  • Active California Occupational Therapist license
  • Current CPR/BLS certification
  • Reliable transportation and willingness to travel to patient homes

Nice To Haves

  • Home Healthcare experience
  • Experience documenting in the Kinnser/Wellsky EMR preferred, but not required

Responsibilities

  • Provide in-home occupational therapy evaluations and treatments according to the physician’s plan of care
  • Assess patients’ strength, coordination, mobility, and ability to function safely at home
  • Develop and update individualized treatment plans based on patient goals and progress
  • Help patients improve independence with daily activities such as dressing, bathing, mobility, and home safety
  • Educate patients and caregivers on exercises, adaptive techniques, and safe movement
  • Document evaluations, visits, and progress in WellSky/Kinnser in a timely manner
  • Communicate patient progress, concerns, and changes in condition to the care team and physician
  • Supervise COTAs in accordance with state guidelines
  • Participate in care planning, case conferences, and discharge planning
