Join Eddy SeniorCare PACE, a nationally recognized model of care that keeps seniors living safely in their homes and communities. We provide a team-based model of care including physicians, RNs, LPNs, Home Health Aides, physical/occupational therapists, social workers, transportation, a day center team, and more! We are privileged to serve patients who are 55 years of age and older, helping them delay/avoid nursing home placement and remain living in the community as long as possible. Currently most of our patients reside at the Sisters of St. Joseph; we also care for patients in the surrounding community. This is a truly unique program our region is fortunate to have access to, with only 130 PACE programs across the country! Occupational Therapist Part-Time Eddy Senior Care-Latham, New York Eddy Senior Care PACE is seeking a dedicated Occupational Therapist to provide evaluation, treatment planning, and therapeutic interventions for participants. Requirements

• NYS Occupational Therapist Licensure.
• Minimum 2 years experience as an Occupational Therapist, with at least 1 year working with older adults.
• Strong clinical judgment, communication, and organizational skills
• Proficiency with EMR systems and Microsoft Office.
• Valid Driver's License and Reliable Vehicle.
• Good driving record

Nice To Haves

• NBCOT certification a plus.
• Experience in PACE and home health preferred

Responsibilities

• Improving functional independence, safety, and quality of life through individualized care plans.
• Collaborate with interdisciplinary team members to ensure integrated care planning.
• Conduct comprehensive evaluations and develop individualized treatment plans.
• Implement therapy programs focused on functional independence and occupation-based activities.
• Educate participants and caregivers on adaptive equipment, home exercise programs, and safety strategies.
• Prepare timely and accurate documentation in compliance with regulatory and organizational standards.
• Participate in team meetings, quality improvement initiatives, and in-service education.
• Coordinate ordering and management of durable medical equipment (DME) as needed.
