About The Position
Completing home visits to evaluate current and/or potential consumers to determine eligibility to receive Personal Care Attendant services at home. Successful evaluations will include: identifying consumers’ needs and functional capabilities, evaluation of the consumer’s home environment along with written cost-effective recommendations on equipment, services, therapies, additional skills training, and community resources that will benefit the consumer to maximize their safety and independence.
Requirements
- Current OT license
- Experience with home health
- Experience with persons with disabilities in the community
- Reliable transportation
Responsibilities
- Home Visits in Hampden, Hampshire, and Franklin county
- Requires evaluations and other documentation to be completed per guidelines set forth by MassHealth and the needs of the organization
