Requirements

  • Graduate of an accredited Occupational Therapy program
  • Active, unrestricted OT license in Illinois
  • Minimum 1 year of clinical experience
  • Willingness to travel within the Des Plaines coverage area
  • Reliable transportation, valid driver’s license & proof of auto insurance
  • Ability to lift 50–100 lbs and perform physical patient care tasks

Responsibilities

  • Conduct comprehensive OT evaluations to determine patient needs and rehab goals
  • Develop individualized, high‑quality Plans of Care
  • Complete OASIS assessments and provide ongoing re-evaluations
  • Communicate patient changes to the care team and physician
  • Promote patient safety using family and community resources
  • Verify and follow physician orders each visit
  • Write orders for additional visits or Plan of Care adjustments
  • Document therapy interventions aligned with goals
  • Provide hands‑on OT services using best practice standards
  • Teach ADLs, fine motor and sensory integration strategies using adaptive equipment
  • Assist with acquiring or fabricating custom splints when needed
  • Provide home exercise program training to patients/caregivers
  • Offer education and supportive counseling to patients and families
  • Supervise COTAs, HHAs, and clinical team members as needed
  • Work in a physically active and occasionally high‑stress environment
  • Travel within the designated service area (up to 50%)
