About The Position

The Occupational Therapist provides professional occupational therapy services to home health clients in accordance with physician orders, the established plan of care, and applicable state practice acts and licensure requirements. This role conducts comprehensive assessments, develops and implements individualized treatment plans, coordinates care with the interdisciplinary team, and educates patients and families to promote safety, independence, and optimal functioning in the home setting. The Occupational Therapist functions with minimal supervision, maintains accurate and timely documentation, communicates changes in patient condition, and supports quality and compliance initiatives. Responsibilities include supervising COTAs and HHAs, participating in care coordination, case conferences, and quality improvement activities, and adhering to organizational policies, ethical standards, and regulatory requirements.

Requirements

  • Graduate of an accredited Occupational Therapy program (AMA/AOTA)
  • NBCOT certified or eligible, with current state licensure/registration to practice as an Occupational Therapist
  • Minimum one (1) year of clinical experience or completion of a 6–8 week preceptorship
  • Ability to work independently with minimal supervision while maintaining professional relationships with patients and families
  • Committed to maintaining clinical competency, agency policies, and professional standards
  • Valid driver’s license and reliable transportation required
  • Demonstrated commitment to the organizational mission, Code of Ethics, and Compliance Program
