We are seeking an experienced Home Health Occupational Therapist to provide patient-centered care in the home setting. This role is ideal for an OT who values autonomy, meaningful patient relationships, and collaboration with an interdisciplinary care team.
Requirements
- Graduate of an accredited OT program
- Active Maryland OT license
- Minimum 3 years of clinical experience (home health preferred)
- Strong assessment and care planning skills
- Reliable transportation & valid driver’s license
Nice To Haves
- EMR experience preferred (WellSky/Kinnser or HCHB)
Responsibilities
- Perform in-home OT evaluations and reassessments
- Develop and implement individualized treatment plans
- Provide skilled interventions to improve functional independence
- Educate patients and caregivers on safety and home programs
- Monitor progress and adjust plans as needed
- Complete accurate and timely documentation
- Collaborate with physicians and care team members
- Supervise Certified Occupational Therapy Assistants (COTAs)
- Maintain compliance with Medicare, HIPAA, and agency standards
