About The Position

The Occupational Therapist (OT) delivers high-quality occupational therapy services to help patients regain independence and improve their ability to perform daily activities. In this role, you’ll assess patient needs, develop individualized treatment plans, and implement interventions that align with Health at Home’s standards and regulatory requirements. Your focus will be on providing compassionate, evidence-based care that enhances quality of life and promotes functional independence.

Requirements

  • Graduate of an accredited Occupational Therapy program.
  • Active, unencumbered OT license in the state(s) of practice.
  • Current CPR certification by an organization-approved provider.

  • One year of clinical experience in home health or related setting.

Responsibilities

  • Assess and Plan: Conduct comprehensive evaluations of patient functional abilities and develop individualized treatment plans.
  • Implement Interventions: Provide occupational therapy treatments to improve self-care, mobility, and adaptive skills.
  • Educate and Support: Instruct patients and caregivers on therapeutic activities, safety, and use of adaptive equipment.
  • Collaborate with Care Team: Communicate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated care.
  • Document Accurately: Complete timely and accurate documentation of evaluations, treatments, and patient progress per organizational and regulatory standards.
  • Assist with Equipment: Recommend and train patients in the use of assistive devices and adaptive equipment.
  • Participate in Meetings: Attend care conferences, in-services, and team meetings as required.
  • Maintain Competency: Demonstrate ongoing proficiency in OT skills and techniques through continuing education and compliance with state licensure requirements.
