Victorian Post Acute is a beautiful 90-bed facility located in a prime area with ample parking. We are a rehab-focused team serving both short-term and long-term patients. We are currently seeking a passionate and motivated Occupational Therapist to join our dynamic in-house therapy team. Why Join Us? Requirements

  • Valid Occupational Therapist License (State of California)

Responsibilities

  • Develop and implement effective treatment plans in coordination with referring physicians.
  • Provide direct therapy services according to the approved plan of care.
  • Train and support Restorative Aides within the nursing department.
  • Supervise Occupational Therapy Assistants (OTAs) in patient care and documentation in line with state practice guidelines.
  • Collaborate with supervisors and interdisciplinary team members to monitor patient progress and adjust treatment as necessary.
  • Participate in care conferences, utilization reviews, and rehabilitation meetings as required.
  • Maintain accurate and timely documentation, including evaluations, daily notes, progress reports, and recertifications according to regulatory standards.
  • Educate patients, families, and caregivers on home programs and discharge planning to promote continued independence.
  • Recommend and coordinate the use of durable medical equipment (DME) to support safe mobility and function (e.g., braces, walkers, grab bars).
  • Promptly report equipment issues to ensure a safe and functional work environment.
