About The Position

As an Occupational Therapist, you will play a critical role in helping individuals of all ages regain independence and improve their quality of life through therapeutic interventions. You will assess patients' physical, emotional, and cognitive abilities to develop personalized treatment plans that address their unique rehabilitation needs. Your work will involve collaborating with multidisciplinary teams to support patients recovering from conditions such as stroke, traumatic brain injury, cerebral palsy, and orthopedic injuries. You will also provide education and support to patients and their families to facilitate ongoing progress outside of clinical settings. Ultimately, your efforts will empower patients to achieve greater functional autonomy and participate more fully in daily activities and community life.

Requirements

• Valid Occupational Therapy License
• 1 year of occupational therapy experience preferred
• CPR certification
• Valid Driver’s License & Auto Insurance
• HomeCare HomeBase experience preferred

Responsibilities

• Perform clinical assessment; identifies patient needs and appropriateness of patient.
• Documents, prepares, revises, and completes a Plan of Care, progress notes, phone orders and other clinical record documentation on an ongoing basis and in a timely manner.
• Accepts clinical assignments that are consistent with education and competence to care for patients/clients.
• Participates in the IDG care planning process.
• Apply concepts of infection control and standard precautions in coordination/performing patient care activities to prevent contamination and transmission of disease.
• Assess and facilitate through direct therapeutic intervention the patient's ROM, strength, coordination, need for and use of adaptive equipment and environmental modifications, functional mobility and safety issues as related to patient’s ADL performance needs. Fabricate and maintain adaptive equipment and hand splints.
• Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
• Evaluate patient occupational performance in the areas of self-care, work, leisure and rest. Assess sensory-motor neuromuscular, psycho-social and cognitive-perceptual components of occupational behavior. Assess physical, social and cultural components of the patient’s occupational environment.
• Meets mandatory continuing education requirements of the Agency and the licensing board.
• Promotes the Agency philosophy and administrative policies to ensure quality of care.
