Requirements
- Active Colorado Occupational Therapist license (required)
- Strong clinical judgment and comfort working independently
- Excellent communication and time-management skills
- Reliable vehicle, valid driver's license, and auto insurance
- A genuine passion for helping people maintain independence and dignity
Nice To Haves
- Home health experience preferred, not required
Responsibilities
- Deliver personalized OT services in patients' homes
- Develop functional, individualized treatment plans
- Complete home safety evaluations and recommend adaptive equipment
- Collaborate with an interdisciplinary care team (nursing, PT, caregivers, families)
- Document care efficiently with full EMR training and support
- Travel locally within Mesa County (mileage and drive time reimbursed)
