Requirements

• Active OT license in Virginia
• 1+ year of home health or adult rehab experience
• Reliable vehicle and valid driver’s license
• Current CPR certification

Nice To Haves

• Familiarity with WellSky/Kinnser EMR preferred

Responsibilities

• Evaluate home health patients for functional and ADL needs
• Build and adjust individualized treatment plans
• Deliver therapy visits in patient homes (Chesapeake area)
• Coordinate with PT, RN, and intake teams for care continuity
• Chart and track visits in WellSky (Kinnser) EMR
• Ensure compliance under PPS/Medicare guidelines
• Support strong patient outcomes through efficient visit use
