Position Summary: The Occupational Therapist is responsible for assuring that the PNHS values of Head + Heart, together, are achieved through daily departmental operations. This includes, but is not limited to, direct patient care, professional growth and development, outcome/compensatory assessments, compliance with regulatory standards and participating in process improvements to advance patient care outcomes and satisfaction. The Occupational Therapist will communicate in a timely and professional manner with staff, patients, patient family members, physician, and other internal/ external customers. The staff will demonstrate skills in listening, conflict management, system problem solving, decision making and process improvement. Therapists will practice in compliance with professional standards of practice and professional code of ethics. The Homecare Occupational Therapist acts as a case manager to assess and evaluate patient care needs and provides skilled therapy care and teaching to patients and families in their home. The Occupational Therapist is responsible to improve and prepare patients and families for self-directed and/or assisted care in their community setting. The Occupational Therapist coordinates medical, social, and financial resources to develop a comprehensive plan of care. The therapist supports the following community care processes: admission, discharge, referrals to other disciplines, billing, and care delivery. Work Schedule: M-F 8:00a-4:30p. Potentially occasional Saturdays. Central Region.
