Requirements

  • Arizona-licensed Occupational Therapist (OT)
  • CPR certification (AHA-approved)
  • At least 1 year of clinical experience (Home Health experience preferred)
  • Organized, compassionate, and reliable
  • Valid driver’s license, auto insurance , and reliable transportation
  • Strong communication skills and clinical decision-making

Nice To Haves

  • Home Health experience preferred

Responsibilities

  • Regain independence through customized therapy plans
  • Improve functional abilities for activities of daily living (ADLs)
  • Recover from injury, surgery, or hospitalization in the comfort of home
  • Reduce fall risk and improve home safety
  • Performing in-home OT evaluations and reassessments
  • Creating individualized care plans tailored to each patient’s goals
  • Collaborating with nurses, physical therapists, and physicians
  • Educating patients and caregivers on proper techniques and safety
  • Documenting patient progress in compliance with state and Medicare guidelines
