About The Position

We are seeking a compassionate and skilled Occupational Therapist to join our home health care team. In this role, you will support patients in regaining their independence and improving their quality of life within the comfort of their homes.

Requirements

• Current and valid state license or certification as an Occupational Therapist.
• Previous experience in case management is preferred.
• Strong interpersonal and communication skills to build trust and rapport with patients and families.
• Ability to work independently and effectively within a collaborative team environment.
• Excellent critical thinking and problem-solving skills with a patient-centered approach.

Responsibilities

• Conduct comprehensive assessments to evaluate patients’ physical, emotional, and cognitive abilities.
• Develop and implement individualized treatment plans tailored to each patient’s unique needs and goals.
• Deliver effective occupational therapy interventions aimed at enhancing patients' daily functioning and independence.
• Monitor, evaluate, and document patient progress, making adjustments to treatment plans as necessary.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure coordinated, high-quality care.
• Educate patients and their families on therapeutic exercises, adaptive strategies, and techniques to support continued progress at home.
