Requirements

  • Graduate of an accredited Occupational Therapy program
  • Active, unrestricted OT license in Indiana
  • Minimum 1 year of clinical experience
  • Ability to travel up to 50% within the service area
  • Reliable vehicle, driver’s license & auto insurance
  • Comfortable lifting 50–100 lbs and performing physical patient care tasks

Responsibilities

  • Conducting initial OT evaluations & ongoing reassessments
  • Creating and updating individualized Plans of Care
  • Ensuring compliance with OASIS & regulatory guidelines
  • Communicating clinical updates to physicians & care teams
  • Teaching ADL techniques, safety strategies & adaptive equipment use
  • Recommending or helping fabricate assistive devices and splints
  • Educating patients & families on home programs
  • Supervising COTAs, HHAs, and clinical staff
  • Traveling within your assigned territory (Merrillville, Portage & Wheatfield)
