About The Position

Klarus Home Health is seeking a Licensed Occupational Therapist (OT) to join our growing home health team in Albuquerque, NM. This PRN/Per Diem role offers one-on-one patient care, flexible scheduling, and the opportunity to work independently while fully supported by a collaborative clinical team.

Requirements

• Bachelor’s degree in occupational therapy
• Current New Mexico OT license
• Valid driver’s license, reliable transportation, and auto insurance
• Current CPR certification

Nice To Haves

• 1+ year OT experience
• Home health experience
• Homecare Homebase proficiency
• Strong communication skills

Responsibilities

• Perform OT evaluations and ongoing assessments, including OASIS as required
• Develop, implement, and revise individualized plans of care
• Provide therapeutic interventions for ADLs, ROM, strength, coordination, and adaptive equipment
• Educate patients, families, and caregivers
• Maintain timely and accurate documentation
• Collaborate with physicians and interdisciplinary teams
• Supervise COTAs and aides as applicable
• Follow infection control and safety standards
